New Patient Info
Dear New Patient and Family:
Below are several links to information for our new patients, which should answer some commonly asked questions about our practice. We want your treatment and care to be as efficient and problem-free as possible.
With that in mind, please do the following:
- Fill out the New Patient History Form as completely as possible, and bring it with you at your first visit.
- Fill out the Patient's Form indicating Patient's Insurance and bring it with you at your first visit.
- Fill out Office Policy for No-Show Appointments and bring it with you at your first visit.
- Fill out the Privacy and Consent of Patient Records (HIPAA) and bring it with you at your first visit.
- Obtain the pertinent lab tests which were requested for your first visit. Please have the lab tests done approximately 1-2 weeks prior to that visit, to ensure up-to-date information on your condition. Please check with your insurance before having test done to be advised if they will be paid for by your insurance. Also ask your insurance company where blood work should be done.
